WELCOME TO SOUTH HAMS AREA WELLBEING

The story so far

South Hams Area Wellbeing or SHAW is a not-for-profit organisation that aims to help enable and educate local individuals to be resilient in their own wellbeing.

It is all about compassionate communities connecting local people of all generations with each other and with organisations, practitioners and agencies that can support and facilitate resilience with the main aim of improving lifestyle, knowledge, happiness and health.

PRESCRIPTION DELIVERY SERVICE

There is a prescription delivery service now available, set up by members of the Livewell team at Kingsbridge Hospital, who have a bank of volunteers to assist with this service. Pharmacies currently covered are;

Well Pharmacy, The Quay, Kingsbridge

The pharmacy at Chillington Surgery.

PLEASE NOTE: It is completely the patient’s responsibility to contact their pharmacy and ask for their prescription to be added to the delivery service.

Pharmacies are attended daily by a volunteer to deliver the prescriptions, but please ensure you allow enough time in order to receive your prescription.

Social prescribing – how it works

You can contact us through your GP surgery or other appropriate healthcare professionals. Our community connectors will arrange an appointment with you to discuss your personal aims and goals.

It’s about what matters to you, not what’s the matter with you.

The connector will introduce you to appropriate individuals, activities, organisations and will then have a follow-up support meeting with you a few weeks later.

Further meetings will be set up as needed with a view to you being self sufficient within an agreed timeframe.

  • Introductions..

    We will introduce you to appropriate individuals, activities, organisations.

  • Follow up and support

    We will then have a follow-up support meeting with you a few weeks later with further meetings as needed.
