Data capture to aid our funding goals

In order for SHAW to achieve its goal and benefit the South Hams community, we will need to demonstrate to our potential funding sources that we are making a difference and enhancing people’s lives.

Consequently, we will be reducing hospital admissions and medicine prescriptions and saving the NHS money, and encouraging our potential funding sources to fund us in the future.

We will therefore be asking patients to answer a couple of appropriate wellbeing questionnaires. These will be held in the strictest confidence. See GDPR.

General Data Protection Regulation (GDPR)

Any information that you give to us will be held safely and securely either as paper records stored in a lockable cabinet in a lockable room and/or securely on a PC or laptop.

We will not share your information with anyone without your permission. Examples include liaising with GPs, other health professionals and community groups.

We will not use your personal information for marketing purposes and we will never give your personal details to a third party unless agreed by yourself. See above.
